MSHSAA Physical Forms

Each student in grades 7-12 who are interested in playing sports must have a physical on file with the school. Physicals are good for 2 calendar years, unless otherwise noted by the student's physician. Print out the form on this page or stop by the school for a copy.  The MSHSAA Medical Eligibility form (Step 3) is the only portion that must be returned to the school.
